the producer survey and the W9 form
(below) and return it to us along with
a copy of Agency License and Errors
and Omissions Coverage.
**All documents must show the same
name for the agency to be appointed,
no individual licenses will be
accepted.**
We are appointing agents in Texas that
meet these guidelines:
Writes 2 million total agency premium
in metro areas or 1.5 million in
cities with a population of 50,000 or
less over the past three years.
Writes 50% commercial business
One standard market
Agency established for at least 2
years

directions.
Complete the Word
Document form **Please provide the
identification number used to acquire
your agency state license. Either your
SS# or Federal Tax ID#.
Print the completed document on your
agency letterhead
